Criteria for Admission
Diagnosis of a terminal illness with a prognosis of three months or less.
Must be enrolled in hospice or be determined to be hospice appropriate and have a DNR order in place.
Must understand that The Heart of Jesus Home for the Dying is not a medical facility, but a home staffed primarily by trained volunteers both male and female who provide care as a family would for their loved one. We do not provide skilled nursing or medical services.
We provide shelter, meals, personal care and 24-hour support for the physical, emotional, and spiritual needs of residents with terminal illness.
No extraordinary care needs such as ventilators and trachs or difficult behaviors that cannot be managed in a safe and comfortable way by a staff of volunteers can be admitted.
Residents and family members must be willing to meet with the Executive Director to determine appropriateness of placement at The Heart of Jesus Home for the Dying.
Final selection is not determined by age, color, creed, ethnic or religious background but by a client who meets the admission criteria and presents the most immediate and significant need and are in agreement with our mission and values.
